  PREVALENCE AND RISK FACTORS OF UNRUPTURED INTRACRANIAL ANEURYSMS IN ISCHEMIC STROKE PATIENTS: A GLOBAL META-ANALYSIS, Descrição: Unruptured intracranial aneurysms (UIAs) have an estimated global prevalence of 2.8 in the adult population; however, UIA was identified among more than 10 of ischemic stroke patients. Many epidemiological studies and reviews have pointed to the presence of UIA among patients with ischemic stroke; yet, the extent of this association is not fully known. We performed a systematic review and meta-analysis to determine the prevalence of UIA in patients admitted to hospitals with ischemic stroke and transient ischemic attack (TIA) at both global and continental levels and evaluate factors associated with UIA in this population. We identified, in five databases, all studies describing UIA in ischemic stroke and TIA patients between January 1, 2000, and December 20, 2021. Included studies were of observational and experimental design. Our search yielded 3581 articles of which 23 were included, with a total of 25,420 patients. The pooled prevalence of UIA was 5 (95 confidence interval [CI] = 46) with stratified results showing 6 (95 CI = 49), 6 (95 CI = 57), and 4 (95 CI = 25) in North America, Asia, and Europe, respectively. Significant risk factors were large vessel occlusion (odds ratios [OR] = 1.22, 95 CI = 1.011.47) and hypertension (OR = 1.45, 95 CI = 1.241.69), while protective factors were male sex (OR = 0.60, 95 CI = 0.530.68) and diabetes (OR = 0.82, 95 CI = 0.720.95). The prevalence of UIA is notably higher in ischemic stroke patients than the general population.   THE EXCITE-BA TRIAL: EFFECTS OF PHYSICAL ACTIVITY PLUS BEHAVIORAL ACTIVATION ON PRESERVING COGNITIVE FUNCTION IN ELDERLY PATIENTS WITH MILD COGNITIVE IMPAIRMENT, Descrição: World population has an increased risk of developing basic neurological deterioration and mild cognitive impairment (MCI). Currently, there is no cure or disease-modifying drug for MCI. Thus, non-pharmacological interventions such as physical activity (PA) and behavioral activation (BA) arise as potential treatment options. This study protocol assesses if a program of combined PA and BA (PABA) reduces disease progression in patients over 60 years old with MCI. This is the protocol for a randomized, controlled, phase II, parallel, single-blinded, multicenter, superiority trial with two groups. In total, 184 patients will be randomized at a 1:1 ratio and stratified by age, center, and level of education via permuted block sizes to receive: 1) a combination of moderate-to-high in-tensity PABA or 2) sham intervention (low-intensity PA) plus psychological support (active control). The primary outcome will be a change in cognitive function assessed by Montreal Cognitive Assessment (MoCA) from baseline to 12 months. Secondary outcomes include changes in MoCA at 3, 6, and 9 months. Furthermore, at 0, 3, 6, 9, and 12 months, subjects will be tested with the Barthel Index, Short-Form 12 Health Survey (SF-12), and Geriatric Depression Scale (GDS-15). This study protocol aims to refine treatment options for MCI and uncover directions for different therapeutic approaches. This will be the first randomized clinical trial to establish the efficacy of PABA compared to active control in the progression of MCI.   FEASIBILITY OF USING DATA COLLECTED THROUGH THE ST. JUDE PEDIATRIC ONCOLOGY FACILITY INTEGRATED LOCAL EVALUATION (PROFILE) TOOL TO DETERMINE PREVIOUSLY DEFINED LEVELS OF PEDIATRIC HEMATOLOGY-ONCOLOGY CARE, Descrição: Pediatric hematology and oncology (PHO) facilities are not all equally equipped to diagnose and treat the full range of childhood cancers. Proposed facility tier models have been published to facilitate the context-based adaptation of PHO diagnostic and treatment guidelines. However, a tool to assist facilities in identifying their level of PHO care does not exist. We assessed the feasibility of using the St. Jude Pediatric Oncology Facility Integrated Local Evaluation (PrOFILE) tool to identify (1) facility levels as defined by the International Society of Pediatric Oncology (SIOP) and the Lancet Oncology Commission (LOC) for sustainable care for children with cancer, and (2) treatment strata in the Adapted Resource and Implementation Application (ARIA) Guide. This study employed a multi-step mapping approach. We mapped the 1,279 PrOFILE tool variables to the nine SIOP service lines, ten LOC domains, and ten ARIA resource groups. Mapping was performed by two teams and carried out in two different rounds. The first round aimed to define if an indicator was fully or partially covered by PrOFILE. Multiple PrOFILE variables could be assigned to each concept to meet its content specificity. Various cut-offs were applied when necessary to assign mutually exclusive responses to each tier. The second round consisted of expert validation for each indicator previously identified. Most (97%) of SIOP?s indicators were partially or fully covered in the PrOFILE tool. Radiation therapy tools and appointment scheduling and call back system could not be mapped. Of the LOC indicators, 89% were covered. Pathology technical staff, chemotherapy complexity (low, moderate, and high), lymph node biopsy, organ preservation, liver transplantation, intensity-modulated radiotherapy, and research infrastructure could not be mapped. Finally, 77% of the indicators used by the ARIA Guide were covered. The ophthalmology resource group could not be fully mapped. This exercise demonstrates the feasibility of using PrOFILE tool variables to identify facility PHO care levels as defined by SIOP and LOC. The tool also contains most of the variables necessary to apply the ARIA Guide to diagnose, treat, and manage childhood cancer.   EFFICACY OF AN ADJUVANT LACTOBACILLUS RHAMNOSUS FORMULA IN IMPROVING SKIN LESIONS AS ASSESSED BY PASI IN PATIENTS WITH PLAQUE PSORIASIS FROM A UNIVERSITY-AFFILIATED, TERTIARY-REFERRAL HOSPITAL IN SÃO PAULO (BRAZIL), Projeto certificado pelo(a) coordenador(a) Marcelo Jenne Mimica em 27/07/2023., Descrição: Psoriasis is an inflammatory disease of the skin, characterized by erythematous plaques. It is rather common, affecting 24 of the population in western countries. Psoriasis etiology encompasses both genetic and environmental factors. Evidence suggests that the latter reflect the importance of changes in the microbiome for developing the disease. Thus, it is hypothesized that gut microbiome manipulation may arise as a way of treating psoriasis. However, few trials assessed the use of probiotics in psoriasis, although promising results were detected in small studies. Our objective was to assess the efficacy of adjuvant probiotics (Lactobacillus rhamnosus) in treating plaque psoriasis patients. This was a randomized, parallel, placebo-controlled, double-blind trial with two arms: experimental (n=50) and control (n=53). Inclusion of subjects and data gathering lasted from November 2020 to August 2021. Subjects were consecutive plaque psoriasis patients under regular follow-up in the Dermatology unit of a university-affiliated, tertiary-referral hospital in São Paulo (Brazil). Eligibility criteria included being over 18 years old, having plaque psoriasis and not having other skin diseases, neoplasms nor systemic inflammatory diseases. Subjects received standard-of-care plus probiotics (Lactobacillus rhamnosus formula). Controls received standard-of-care plus placebo. Primary outcome was skin lesion improvement as assessed by psoriasis area of severity index (PASI) at six months. Secondary outcome was quality-of-life as assessed by dermatology life quality index (DLQI) at six months. Regarding within-group analyses, changes in both PASI and DLQI were non-significant for the experimental group (mean PASI decreased by 1.58, p=0.105, and mean DLQI increased by 0.05, p=0.873) and significant for controls (mean PASI decreased by 1.90, p=0.019, and mean DLQI decreased by 3.33, p=0.031). Between-group analyses returned non-significant results (p=0.620). Our findings do not support the hypothesis that gut microbiome modulation via ingestion of Lactobacillus rhamnosus produces clinical improvement in psoriasis patients.   MAPPING PROFILE TO SIOP LEVELS OF CARE: PROFILE RESULTS CAN ASSIST TEAMS IN IDENTIFYING THEIR CURRENT SIOP FACILITY LEVEL OF CARE, Descrição: The International Pediatric Oncology Society (SIOP) Pediatric Oncology in Developing Countries (PODC) Committee has a framework to adapt treatment regimens, which outline definitions for pediatric hematologyoncology (PHO) levels of care (04). We aimed to assess if the Pediatric Oncology Facility Integrated Local Evaluation (PrOFILE) tool can assist teams in identifying their SIOP facility level of care. We mapped PrOFILE Full Version questions to the nine SIOP levels of care service lines and corresponding 65 concepts in three steps. First, we identified variables available in the PrOFILE tool that answer each SIOP concept. Then, we determined the cut points (PrOFILE scores) to define the SIOP level. We considered that a concept was fully covered by PrOFILE when all the elements defined in the SIOP concept were available and SIOP categorization throughout the four levels was possible. A partially covered concept did not allow categorization throughout all levels. Finally, the SIOP concepts were applied to five realcase scenarios to test the face validity and feasibility of the predefined cutpoints. A total of 263 out of 1279 PrOFILE variables were used to apply the SIOP levels. Most of the concepts were fully covered (57%) or partially covered (38%). The latter were in the following service lines: Personnel (10), Diagnosis and Staging (4), Logistical and Social Support (4), Health System (2), Medical Facilities (2), Supportive Care (2) and Access to Medications (1). Three SIOP concepts (5%) were not available for classification: Appointment Scheduling and CallBack System, Radiation Therapy Personnel and Radiation Therapy Planning Tools. Applying the scoring system to existing data from five institutions was feasible and the assigned SIOP level was reasonable. We concluded that data collected for PrOFILE can generate a SIOP PHO level of care (04).   COGNITIVE IMPAIRMENT IN ANKYLOSING SPONDYLITIS PATIENTS: A CASE-CONTROL STUDY, Descrição: Ankylosing spondylitis (AS) is a chronic, progressive and inflammatory disease that commonly affects the vertebrae and the sacroiliac joints, causing pain and loss of mobility. Extra-articular manifestations vary widely in terms of both frequency and severity. In addition, recent studies have suggested that neurological manifestations might be more frequent than expected in autoimmune rheumatological diseases. Little is known about the potential systemic effects of AS on the nervous system. The aim of this study was to assess the frequency and the clinical predictors of cognitive impairment in ankylosing spondylitis (AS) patients. A cross-sectional and case-control study was performed including consecutive AS patients seen in a rheumatology outpatient clinic of referral tertiary hospital. The control group included 33 healthy subjects. We registered clinical and demographic data including age, sex, level of education, time of disease, time of diagnosis, drugs in use, cardiovascular risk factors and other comorbidities. Functional capacity was assessed using the Health Assessment Questionnaire (HAQ). Neurological appraisal was made with standardised questionnaires: Brief Cognitive Screening Battery (BCSB), Montreal Cognitive Assessment (MoCA), Clinical Dementia Rating (CDR) and the Hospital Anxiety and Depression (HAD). Analysis of the data was performed using qui-square and t-tests and a multivariate analysis (SPSS 22.0).
