  The impact of social drivers, conditional cash transfers and their mechanisms on mental health of the young: an integrated retrospective and forecasting approach using the 100 million Brazilian Cohort, Descrição: Physical, emotional, and social changes, including exposure to poverty, abuse, or violence, increases youth vulnerability to mental illness. These factors interfere with development, limit opportunities, and hamper achievement of a fulfilling life as adults. Addressing these issues can lead to improved outcomes at the population level and better cost-effectiveness for health services. This should be especially important to low- and middle-income countries, due to the substantial number of financially and socially vulnerable individuals in these settings. Cash transfer programs have been a promising way to address some of the social drivers for poor mental health among youth. It has been shown that such programs can alleviate income restriction, increase schooling and access to health facilities, and decrease alcohol consumption and violence in families. However, it is still unclear which pathways and mechanisms explain the association between socioeconomic support and lower mental illness among youth. In this project, we will evaluate the effect of social drivers on youth mental health-related hospitalizations and suicide, test mechanisms and pathways of a countrywide socioeconomic intervention, and examine the timing of the intervention during the life course. For example, determining whether cash transfers delivered earlier in life can contribute to better outcomes over time. This project will also investigate the cash transfer effect on vulnerable groups (African descendants, women, and youth with lower income). We will evaluate social drivers determination and the association between being beneficiaries of the Conditional Cash Transfer program, called the Bolsa Família Program (BFP), and mental illness and its long-term effects using robust methodologies for big datasets, such as Regression Discontinuity Designs, Propensity Score Matching and difference-in-differences. We will test mechanisms and pathways, between BFP and mental health-related hospitalizations and suicide among youth using Structural Equation Modeling. Furthermore, we will perform microsimulations to generate projections regarding how mental health-related hospitalizations and suicide trends will be in the future based on the current state, and how BFP implementation scenarios will affect these trends. The results of this project will be of vital importance to guide policies and programs to improve mental health and reduce mental health-related hospitalizations and suicide in youth. It will provide information to improve the effectiveness of these programs worldwide. Finally, it can potentially advance the global debate on the economic response to the pandemic and times of economic crises.   Projeto de Pesquisa Avaliação do impacto da expansão do acesso e do fortalecimento da Estratégia Saúde da Família na APS no SUS, Descrição: Apoiar a SAPS/MS nas estratégias de avaliação do impacto da expansão e do fortalecimento da Estratégia Saúde da Família após a publicação da Portaria n 3.493, que institui a nova metodologia de cofinanciamento federal do piso de Atenção Primária à Saúde do SUS. Estimar, mediante técnicas avançadas de microssimulação, os custos e os impactos da cobertura da Estratégia Saúde da Família (ESF) sobre populações sem acesso, considerando efeitos em indicadores de saúde populacional (por exemplo, morbi-mortalidade) e em indicadores relativos aos serviços de saúde (como produtividade).   HEALTH_PROTECT: Implementing interventions to reduce health inequalities and mitigate the impact of economic crises in Latin America, Descrição: Research Project Summary:Principal research question. Socioeconomic inequalities are the predominant cause of health inequalities: which are the most effective economic-based policies to minimise health inequalities and mitigate the health impact of the current economic downturn in Latin America?Aim. To evaluate the impact of the current increase in socioeconomic inequalities on morbidity, mortality and health inequalities in five Latin American countries (LA), namely Brazil, Argentina, Colombia, Ecuador, and Mexico (BACEM - representing more than half of the population of the continent), and to develop an open- access platform able to design and calibrate the most effective economic-based policies in each country for the reduction its health inequalities.Specific Objectives1.To evaluate the impact of the current worsening of the socioeconomic conditions, including income, wealth, and poverty - due to the ongoing global economic downturn - on morbidity and mortality of the population in BACEM, both overall and for age-groups, systematically evaluating the diseases and health conditions most affected.2.To forecast the most probable scenarios of the evolution of income inequalities and poverty rates in BACEM up to 2030, and their impact on poverty-related health outcomes and health inequalities.3.To develop integrated models that will join consolidated economic-based microsimulation with health- based microsimulation to evaluate the impact and cost-effectiveness of economic interventions and poverty- reduction policies on morbidity, mortality, and health inequalities in BACEM.4.To create a platform (named HealthProtect) that will incorporate all developed datasets, impact estimates, microsimulation, and forecasting models and will allow policymakers to simulate and calibrate - through a user-friendly visual analytics dashboard - the most impactful and cost-effective economic policies to improve health and reduce health inequalities in BACEM, including their more efficient implementation strategies (with different temporal and geospatial implementations, and different targeting strategies).5.To apply the integrated microsimulation models of HealthProtect to design the most impactful and cost- effective strategies for the mitigation of the health effects of the current and future economic crisis scenarios on morbidity, mortality and health inequalities in each BACEM.6.To advocate and disseminate the use of HealthProtect in the Ministry of Economics and Ministry of Health in BACEM, by training policymakers to use the open access and user-friendly platform, promoting a collaborative approach between stakeholders for the reduction of health inequalities.II. This study will use one of the largest longitudinal dataset from the linkage of socioeconomic and health data to evaluate the effects of socioeconomic determinants, cash transfer programmes and primary care on the incidence, hospitalization, adherence to treatment and mortality from HIV/AIDS. Innovative mathematical models will be created to forecast the impact of alternative scenarios and identify the best policy options to reduce the burden of the disease.
