Avaliação da replicação residual do HIV-1 em diferentes populações de hospedeiros humanos, Descrição: A síndrome da imunodeficiência adquirida, causada pelo vírus da imunodeficiência humana (HIV), é uma doença crônica responsável pela morte de 2,0 milhões de pessoas no mundo todos os anos. O HIV infecta principalmente linfócitos T CD4+, levando a uma diminuição gradual no número dessas células ao longo dos anos. Além disto, a replicação do HIV leva a ativação de células T CD4+ e CD8+ com conseqüente envelhecimento prematuro do hospedeiro, proporcionando entre outras a aceleração na atrofia encefálica cerebral com conseqüente alterações cognitvas, osteopenia com fraturas patológicas, fenômenos ateroscleróticos, disfunção ventricular esquerda e insuficiência renal e hepática. Alguns indivíduos hospedeiros controlam naturalmente a replicação viral do HIV enquanto outros progridem maios rapidamente em sua doença. A progressão mais acelerada pode estar ainda relacionada a características das variantes virais infectando hospedeiros diferentes. Por sua vez, ainda não é completamente clara a intensidade da supressão viral proporcionada pelo tratamento com medicamentos antirretrovirais. Carga viral e genotipagem do vírus de Epstein-Barr (EBV) e análise da freqüência das variantes do gene viral BNLF-1 em indivíduos portadores do vírus da imunodeficiência humana 1 (HIV-1), Descrição: O vírus de Epstein-Barr (EBV) foi descrito em cultura de células provenientes de um caso de linfoma de Burkitt, tumor comumente observado em crianças da região sub-Saara na África. O EBV está presente em mais de 90% da população mundial, infectando células epiteliais e linfócitos B. O vírus é capaz de imortalizar linfócitos B humanos em cultura celular e está associado ao desenvolvimento da forma endêmica do linfoma de Burkitt e do carcinoma indiferenciado de nasofaringe em 100% dos casos. Adicionalmente é detectado em parcela dos casos de linfomas não-Hodgkin (particularmente em indivíduos imunocomprometidos), do linfoma de Hodgkin clássico e do adenocarcinoma gástrico. A terapia anti-retroviral combinada de alta potência (HAART) contribuiu de modo significativo para diminuição na morbidade e mortalidade em decorrência da infecção pelo vírus da imunodeficiência humana (HIV). A despeito da diminuição observada nos casos de sarcoma de Kaposi após a introdução da HAART, o mesmo não foi verificado em linfomas, cujo risco é aumentado em indivíduos infectados pelo HIV. Ressalta-se que ainda não está claro o impacto desta terapia anti-retroviral na incidência de linfomas em indivíduos portadores do HIV, principalmente os causados por vírus oncogênicos como o vírus associado ao sarcoma de Kaposi/Herpesvírus Humano tipo-8 (KSHV/HHV-8) e o EBV. O presente estudo visa detectar o EBV em amostras de sangue periférico de 100 pacientes portadores do HIV atendidos no Hospital-Dia de HIV/Aids da Faculdade de Medicina de Botucatu, UNESP. Nos casos EBV-positivos será efetuada a quantificação de carga viral pela técnica de reação em cadeia da polimerase em tempo real (qPCR). Adicionalmente esses casos serão analisados quanto ao genótipo viral presente e freqüência das variantes do gene viral BNFL-1, que codifica a oncoproteína LMP-1 do EBV. Estudo prospectivo da infecção pelo herpesvírus associado ao sarcoma de Kaposi / Herpesvírus humano tipo 8 (KSHV/HHV-8) em pacientes portadores do Vírus da imunodeficiência Humana (HIV), Descrição: Pacientes infectados pelo HIV, o agente etiológico da aids, estão sob maior risco de desenvolverem neoplasias malignas associadas à infecção por certos vírus oncogênicos, incluindo o KSHV/HHV-8. Esse vírus é responsável pelo desenvolvimento do sarcoma de Kaposi e determinados linfomas, notadamente em indivíduos HIV-positivos. São descritos cinco grupos genotípicos principais do KSHV/HHV-8, cuja distribuição nas populações humanas é influenciada por fatores etno-geográficos. Entretanto, são raros os estudos sobre a freqüência desses genótipos virais em pacientes portadores do HIV em nosso país. Adicionalmente, não está definido se a infecção por determinados genótipos do KSHV/HHV-8 influencia o risco para o desenvolvimento de neoplasia maligna ou modifica a evolução clínica do paciente portador do HIV. O presente estudo visa avaliar prospectivamente 150 pacientes portadores do HIV-1 que não tenham desenvolvido neoplasia maligna associada ao KSHV/HHV-8. Serão efetuadas coletas de sangue periódicas a fim de se determinar a prevalência de infecção pelo KSHV/HHV-8 e quais os genótipos virais mais freqüentes. Para tanto serão utilizados métodos de Biologia Molecular baseados em PCR e seqüenciamento de ácidos nucléicos.
