Responsible to the Vice President, Aviation Standards and Safety, directly or through the assigned supervisor, in but not limited to the following: Organize and/or develop an accident prevention and investigation office, including the development of an efective aviation accident prevention/investigation program, and the establishment and implementation of a system for the dissemination of information contained in accident/incident reports. Perfrom periodic safety surveys and recommend corrective action. Assist Airport Directors and Airport Safety Coordinators in all matters relating to aviation safety; establish close liaison with airports technical personnel through developing/participating in safety committees and through frequent personal visits. Distribute and post accident prevention data, literature and information to all parties concerned. Coordinate with the Manager of Ground Safety and assist in the investigation of all serious ground accidents. Conduct periodic classes in the principles of aviation safety for airport safety coordinators and other aviation personnel. Maintain files, records, graphs, and charts of all aviation flight and ground-related personnel and equipment accidents and incidents. Participate in accident and incident investigations. Perform other related duties as assigned.

Directs the activities of all Embraer Go-Team members during all phases of an accident or incident investigation, specifically supervising the inspection and analysis of all involved ATA100 systems. Directs all subsequent investigative follow-up actions, including the analysis of FDR, the detailed inspection of all on-site evidence with special attention to the collection and preservation of non-volatile memory and the development of probable cause statements and subsequent recommendations. Provides technical reviews of all Embraer accident and incident reports to ensure completeness, accuracy and adherence to aspects of potential product liability. Leads the Embraer Go-Team during the on-site investigation of major accidents as detailed in the Embraer Aircraft Crisis Management Plan. Maintains direct contact with investigative authorities (IIC) during all phases of an investigation with an emphasis on providing a rapid response to requests for additional investigative support, as well as monitoring progress during all phases of the investigative process and providing updated information to all involved Embraer activities. Conducts internal training on the technical aspects of accident investigation for Go-Team members as well as engineering and technical support personnel who may also become involved in the on-site phase of a major investigation. Reviews manufacturer's accident reports as well as all subsequent updates for completeness, technical accuracy and adherence to all aspects of product liability concepts.

Established and coordinated on-site investigation procedures for major Army-wide mishap investigations; identified items most promising for the development of satisfactory evidence of probable cause. Secured evidence through interviews, visual inspections, laboratory tests and reenactment methods/ analyzed evidence to determine the preliminary probable cause of the mishap in order to develop preliminary safety recommendations. Developed, arranged and monitored additional laboratory studies and functional tests of failed or malfunctioning components in order to isolate the cause. Compiled, drafted and presented formal technical reports concerning specific mishap investigations. At the completion of this assignment, had conducted 23 major mishap investigations. Analyzed the relationship of environmental factors, cockpit configuration and crew workload in all phases of operations; reviewed human factor aspects of mishaps directly involving personnel other than flight crew members and evaluated the adequacy of existing regulations concerning aircraft crashworthiness, emergency response procedures and personal protective equipment. Taught human factors and accident investigation methods and techniques as an instructor in the US Army Safety Center Aviation Safety Officer Course. Directed, reviewed, and revised safety curriculum in professional development courses. Performed safety assistance and evaluation visits for four major Army aviation commands worldwide. Prepared and reviewed Department of the Army level mishap prevention directives. Served as an Army aviator in both rotary wing and fixed wing aircraft.

Prepared independent safety assessment report for Robertson Avaition´s internal auxiliary fuel tank system as part of the certification requirements, established by the U.S Army´s procurement contract. The report was written during August and September 1988. The internal auxiliary tank was for installation in a Model HM-020-185E (UH-60A configuration) helicopter. The report was accepted by Robertson Aviation and the US Army without further comment.
