  Ganglionar Tuberculosis Infection Evolving to Hemophagocytic Lymphohistiocytosis After Anti-Programmed Cell Death 1 Treatment for High Risk Melanoma: Case Report., Descrição: Melanoma is a relatively common cancer. Cutaneous melanoma is the most frequent form and usually has a high mutational load due to carcinogenesis induced by ultraviolet radiation. The impressive data of immunotherapeutic agents in the metastatic scenario led to its utilization in the adjuvant context, and now immunotherapy has been increasingly used worldwide. However, immune-related adverse events are also becoming more frequent and challenging situations may arise. Hemophagocytic lymphohistiocytosis is a rare but potentially fatal syndrome of immune hyperactivation that may be an under-recognized immune-related adverse event. Conditions associated with secondary hemophagocytic lymphohistiocytosis are infections, malignancies or rheumatologic disorders. Here we describe a ganglionar tuberculosis infection evolving to hemophagocytic lymphohistiocytosis in a 76 year-old patient treated with adjuvant nivolumab for high-risk melanoma.Case Presentation A 76-year-old male diagnosed in 2017 with a superficial spreading melanoma pT4bpN1a was treated with adjuvant nivolumab 3mg/kg every 14 days for 1 year. Two months following nivolumab completion, the patient presented with fever, weight loss, diffuse petechiae and splenomegaly associated with anemia, thrombocytopenia, hypofibrinogenemia and hyperferritinemia. Positron emission tomography scan showed significant hypermetabolism in cervical, supraclavicular, mediastinal and abdominal lymph nodes. Bone marrow aspiration demonstrated no alterations, except for a hypercellular pattern. Dexamethasone and intravenous immunoglobulin were started due to the suspicion of hemophagocytic lymphohistiocytosis. Core biopsy of the infracarinal lymph node revealed a chronic granulomatous inflammation and caseous necrosis, with positivity for Mycobacterium tuberculosis by PCR, and treatment for ganglionar tuberculosis was started.Conclusion The present case highlights the challenges involving the clinical use of antiprogrammed cell death 1 agents, especially in the context of high-risk cutaneous melanoma, in which infections, lymphoproliferative disorders, and even sarcoidosis can mimic disease progression. Cases of tuberculosis have been reported during and after anti-programmed cell death 1 treatment. As programmed cell death 1 blockade becomes more globally used in tuberculosis endemic regions, it is possible that tuberculosis-related adverse events in the context of immunotherapy may increase.   NTRK Inhibitors in Non-Small Cell Lung Cancer., Descrição: Care should be taken to ensure that the diagnostic strategy for a recently diagnosed advanced non-small cell lung cancer includes NTRK fusion testing. RNA sequencing is the gold standard method of detection of NTRK fusion, however pan-TRK immunohistochemistry could be used as a screening method with good sensitivity. Larotrectinib and entrectinib are approved therapies for TRK fusion positive lung cancers as first or subsequent lines of therapy. TRK inhibition has demonstrated clinically meaningful, deep and durable systemic and central nervous system responses. Larotrectinib and Entrectinib were very safe, with manageable safety profile, including some TRK related adverse events, such as dizziness and weight gain.   Prolonged Survival with Regorafenib in Metastatic Cholangiocarcinoma: Case Report, Descrição: Advanced cholangiocarcinoma is a rare, aggressive tumor. Treatment is limited and very few systemic therapies are available. An effort for new drugs development are in course, which aims higher response rates, prolonged survival and better quality of life. Currently, immune checkpoint inhibitors and tyrosine kinase inhibitors have been used as new approaches after first or second line chemotherapy. CASE PRESENTATION: A 49-year-old woman, diagnosed in 2013 with an intrahepatic cholangiocarcinoma, underwent surgery (pT2pN1) followed by adjuvant capecitabine and radiotherapy. Disease relapse occurred 9 months later, with a unique hepatic lesion that was treated with gemcitabine and cisplatin following liver ablation by radiofrequency. After 2 years, the identification of metastatic lesions in liver, lymph nodes and bones leads the patient to a re-exposure to gemcitabine and cisplatin, which resulted in partial tumor response, accompanied by limiting myelotoxicity induced by chemotherapy. A combination of fluorouracil, leucovorin and irinotecan was started, but disease progressed after 7 cycles. The patient received regorafenib in a third line setting, which resulted in a substantial metabolic response in all secondary lesions. The patient remains with stable disease for more than 16 months. CONCLUSION: A surprising prolonged survival with regorafenib was observed.   Prevalence of PD-L1 Expression and Common Genetic Alterations in Advanced Non-Small Cell Lung Cancer (NSCLC) in an Oncology Reference Center in Brazil, Descrição: Background: Several tissue processing and storage factors can interfere with PD-L1 evaluation and genetic analysis. Indeed, some reports suggest a lower prevalence of PD-L1 expression in advanced NSCLC in low-income countries, where usually only the most prevalent genetic alterations are tested. Here, we aimed to study the PD-L1 tumor cell expression in NSCLC samples in a Brazilian oncology reference center. Methods: We carried a single-institution retrospective analysis of all the patients (PTS) with NSCLC between 2017 and 2019 at Hospital Sírio-Libanês, whose tumors were tested for PD-L1 expression.
