Ronald Edward Blanton

Eu sou um médico especializado em doenças infecciosas e chefe do Departmento de Medicina da Universidade Tulane Escola de Saúde Pública e Medcina Tropical. Eu tenho mais de 30 anos de experiência em pesquisa sobre infecções endêmicas no Brasil e mais de uma década de experiência de investigação em África também. Eu me formei em literatura em inglés na Universidade Harvard e depois de graduar em medicina. Completei o mestrado em epidemiologia genética em 2003 na Universidade Case Western Reserve. Agora eu sou chefe do departmento de Medicina Tropical de Tulane em New Orleans, Louisiana, USA. Muito da minha pequisa faz ênfase em Schistosoma mansoni, dengue e a resistência bacteriana, bem como alguns trabalhos em Aedes aegypti. A disciplina principal que aplico à estes problemas nos últimos 10 anos tem sido a epidemiologia genética e a genética de populações aplicada à saúde pública. A estrutura de populações de patógenos ou vetores pode revelar como se formou, de onde foram, seu distribuição, o efeito de programas para controle e até mesmo o futuro delas. Outra área de crescente importância para a minha pesquisa tem sido a resistência antimicrobiana e sua relação com a falta de saneamento. Nós começamos estudos que traçam padrões de resistência da casa para a clínica e vice-versa.

    Environmental influences on urban schistosomiasis transmission and elimination, Descrição: Today the great challenge for management of schistosomiasis in Brazil and other countries with low endemicity is to move from morbidity control to elimination. Most efforts see initial reductions in prevalence and intensity followed by low level transmission that constantly threatens to blossom if neglected. Human immigration, the health of the parasite population, potential drug resistance, climatic conditions and the level of human fecal contamination of surface waters can all contribute to persistence. Although schistosomiasis is often considered a rural disease, it is increasingly being identified in large urban areas where 80% of the Brazilian population resides. Elimination of schistosomiasis from an urban area can, however, be a more tractable proposition than a rural area. Urban foci in general have lighter infections than rural areas and thus nearer to elimination as a starting point. In Brazil, urban sanitation is generally better than in rural areas. Thus focal elimination is much more likely in a city where there are "islands of infection." Within the suite of tools for an endgame for this infection ? chemotherapy, snail control, sanitation, vaccination - there should also be instruments for understanding why parasite populations are able to persist after repeated treatments, especially in urban environments. We have developed molecular techniques and analytic approaches that permit us to examine large numbers of fecal samples for parasite allele frequencies. We have shown that this approach can be explanatory for parasite history and discriminatory for populations separated by even short distances. In a rural area we identified the parameters associated with a genetically significant reduction in parasite numbers. Decreased diversity, and by extension population health, did not occur until the overall community parasite burden was reduced by 10 fold. Further, we introduced a highly specific marker for human fecal contamination and showed that its local concentration can be correlated with risk of infection. We have identified active S. mansoni transmission and human infection in one neighborhood of Salvador, Bahia, Brazil and infected snails in other neighborhoods. How the infection is distributed within communities, how the human, parasite and snail populations change over time has implications for how transmission might be eliminated. Each year we will administer questionnaires and perform 3 stool examinations for all residents in 2 neighborhoods where the prevalence of schistosomiasis is 10-20%. Every year those found to be infected will be treated. We will also survey monthly for snail infections, fecal contamination and climatic conditions. Infection incidence and reinfection will be identified. Parasite populations in urban foci of schistosomiasis will be genetically characterized and tracked, and the spatial and longitudinal distribution of human fecal contamination and snail populations will be assessed and related to risk.     The influence of local ecology on waterborne diseases, Projeto certificado pelo(a) coordenador(a) Luciano Kalabric Silva em 22/05/2018., Descrição: Most water-borne diseases are the result of either ingestion or contact with water contaminated with feces from humans and/or animals. In Brazil these diseases can be broadly classified as diarrheas or schistosomiasis. Although the transmission mechanisms may differ, there is a logic and an economy in considering transmission pathways for both conditions together. They find commonality in their origin in the breakdown in sanitation and their consequences on nutrition, growth and cognitive development. Studies that have examined the relationship of quantitative fecal contamination of water to diarrheal disease have produced variable results indicating that the relationship is not a simple one. By contrast, for schistosomiasis, a chronic disease, we have shown that this relationship is quantitatively more direct. Water-transmitted diseases result from the interaction of multiple factors in addition to the degree of fecal contamination.     Recurrent Treatment: The effect on population structure of rural and urban Schistosoma mansoni, Descrição: The study has as its objective to determine how Schistosoma mansoni is maintained in the community and how infection returns following treatment.
