  O efeito da doxorrubicina sobre o tecido adiposo. Elucidando o papel da família dos PPARs., Descrição: O número crescente de casos de câncer tem levado ao aumento gradual no impacto desta doença na nossa sociedade moderna. Hoje o câncer já é a segunda causa de morte no mundo, com perspectivas de ser a principal causa já na próxima década. Apesar dos inúmeros avanços no tratamento, a expectativa e a qualidade de vida do paciente pode ser muitas vezes afetada pelo próprio tratamento quimioterápico. Nosso grupo tem demonstrado que a doxorrubicina, principal quimioterápico utilizado na prática clínica contra tumores sólidos, sendo utilizado isoladamente ou na poli-quimioterapia apresenta efeitos devastadores sobre a regulação metabólica do tecido adiposo, assim como do músculo esquelético. Esses efeitos deletérios podem causar sinais e sintomas semelhantes a caquexia induzida pelo tumor e podem reduzir a qualidade de vida e inclusive aumentar a mortalidade. Por isso, acreditamos ser essencial a investigação de tratamentos coadjuvantes que possam atuar de maneira efetiva e segura na redução dos efeitos colaterais da doxorrubicina sem afetar a eficácia da droga. O tecido adiposo é severamente afetado pela doxorrubicina que promove a inibição do PPAR-gamma. Esta redução é capaz de afetar vários processos metabólicos controlados por este fator de transcrição, como lipogênese, adipogênese, captação de glicose e produção de adipocinas. Além disso, recentemente foi demonstrado que esta droga aumenta a lipólise por um mecanismo dependente de PPAR-alfa. Esses efeitos induzem uma severa lipotoxicidade. Portanto, pretendemos utilizar a pioglitazona, um ativador de PPAR-gamma, assim como um modelo geneticamente modificado, com a deleção parcial de PPAR-alfa para elucidar o papel dessa família de fatores de transcrição como possíveis alvos para redução da lipotoxicidade. Portanto, nosso objetivo é avaliar o papel do PPAR-alfa e do PPAR-gamma sobre a lipotoxicidade gerada por um ciclo (5 semanas/ 3 mg/kg de peso 1X por semana ip) de doxorrubicina. Investigaremos assim o papel do co-tratamento de pioglitazona com o ciclo de doxorrubicina ou a deleção parcial do PPAR-alfa podem mitigar os prejuízos da doxorrubicina sobre a lipólise, lipogênese, adipogênese, captação de glicose, inflamação, adipocinas e o fenótipo dos macrófagos infiltrados no tecido adiposo dos animais tratados.   Efeitos metabólicos da doxorrubicina no músculo esquelético e tecido adiposo. Possível papel adjuvante do treinamento físico e metformina., Descrição: A doxorrubicina é um antibiótico da família das antraciclinas, mais utilizado na prática clínica, no tratamento de um grande número de tumores sólidos. Foi observado que pacientes que utilizam esse medicamento possuem maior risco para o desenvolvimento de insuficiência cardíaca, limitando a eficiência do tratamento. Outro fator preocupante no tratamento com essa droga é a fadiga severa. Trabalhos recentes, vem mostrando que outros tecidos, além do cardíaco também sofrem com a toxicidade da doxorrubicina, dentre eles o tecido adiposo e o músculo esquelético. Dados coletados no nosso laboratório demonstram que a doxorrubicina causa uma hiperglicemia acompanhada de uma grande diminuição da sensibilidade à insulina no músculo esquelético, além de promover uma grande diminuição no conteúdo protéico da proteína quinase ativada por AMP (AMPK) no músculo esquelético, esta enzima é uma importante mediadora do metabolismo oxidativo e da captação de glicose independente de insulina. Faltam estudos evidenciando os danos metabólicos causados pelo tratamento com a doxorrubicina no músculo esquelético e no tecido adiposo, no entanto, o caos metabólico provocado nesses tecidos pelo tratamento com doxorrubicina afetam a qualidade e expectativa de vida dos pacientes, assim como a própria eficiência do tratamento. O objetivo do presente projeto é investigar o papel do tratamento da doxorrubicina, sobre o metabolismo do músculo esquelético e do tecido adiposo, nas vias de captação de glicose, dependentes e independentes de insulina. Propondo co-terapias adjuvantes para inibir ou atenuar os efeitos deletérios da doxorrubicina sobre a captação de glicose e sensibilidade a insulina, e para isso trataremos os animais com dois estímulos que ativam a AMPK, um farmacológico (metformina) e outro não farmacológico (treinamento físico). Os animais receberão seis doses de doxorubicina intraperitoneal, a cada 72 horas, com uma dose de 2,5 mg/kg do peso corporal. Os animais serão divididos em: grupo controle (sem administração de nenhuma droga e sedentários, grupo doxorrubicina (animais que serão tratados com a doxorrubicina), grupo doxorrubicina mais metformina (droga hipoglicemiante, agonista da AMPK) ou grupo doxorrubicina associado ao treinamento (onde os animais se submeterão a sessões de exercício físico 5X na semana, com intensidade 50% do Vo2 máx, assim que derem início ao tratamento quimioterápico).
