Felipe Fregni
Full Professor de Medicina Física Reabilitação da Escola de Medicina de Harvard (Harvard Medical School) e Full Professor de Epidemiologia da Escola de Saude Publica de Harvard. Professor de Pós-Graduação do Núcleo de Neurociências (NEC) da Universidade de São Paulo (USP) e São Paulo Excellence Chair/USP (SPEC - FAPESP). Diretor do Laboratório de Neuromodulação do Hospital de Reabilitação Spaulding (Spaulding Rehabilitations Hospital). Possui doutorado em Psiquiatria pela Universidade de São Paulo (2004), mestrado pela Escola de Medicina de Harvard, mestrado pela Escola de Saúde Pública de Harvard (Harvard School of Public Health) e pós-doutorado pela Universidade de Harvard. Tem experiência na área de Medicina, com ênfase em Psiquiatria e Neurologia, atuando principalmente nos seguintes temas: estimulação transcraniana por corrente contínua, estimulação magnética transcraniana, plasticidade cerebral e metodologia em pesquisa clínica com aplicação em saúde pública.

Pragmatic Trial of Remote tDCS and Somatosensory Training for Phantom Limb Pain with Machine Learning to Predict Treatment Response, Descrição: A dor do membro fantasma é considerada um distúrbio extremamente difícil de tratar, uma vez que os tratamentos tradicionais não são eficazes no direcionamento dos circuitos neuronais desadaptativos associados à dor crônica. A estimulação transcraniana por corrente contínua (ETCC) é uma técnica de estimulação cerebral não invasiva e segura que demonstrou reverter a plasticidade mal adaptativa, bem como reduzir a dor na dor neuropática e outras síndromes dolorosas. Nossos objetivos específicos deste projeto testarão as seguintes hipóteses: (Objetivo 1) ETCC combinada com terapia somatossensorial associada a um tamanho de efeito significativamente maior (de pelo menos 1) em comparação com a condição de controle na redução da dor; (Objetivo 2) identificar preditores de resposta deste tratamento combinado usando algoritmos de aprendizado de máquina ajudará a identificar diferentes fenótipos de dor em pacientes com PLP e melhorar seu tratamento alvo; (Objetivo 3) o tratamento combinado irá reforçar o tônus #8203;#8203;parassimpático (conforme indexado pela maior variabilidade da frequência cardíaca) e reduzir a ativação simpática, alterações que serão correlacionadas com diminuições de PLP. Esta contribuição é significativa porque, embora vários estudos tenham testado a eficácia da ETCC para a dor crónica, há necessidade de avaliar a sua eficácia num cenário do mundo real, e esta proposta fornece dados críticos para desenvolver uma intervenção segura e única a ser aplicada. em casa, o que pode, portanto, aumentar o seu acesso a populações sub-representadas e diminuir os custos terapêuticos. Mechanisms of Open and Hidden Placebo in Stroke Recovery, Descrição: Propomos investigar os mecanismos neurais do placebo (aberto e oculto) em indivíduos com AVC. Esta proposta baseia-se no nosso ensaio clínico recentemente concluído NIH R21 (R21HD079048-01A1), que mostrou que o efeito placebo que contribui para a recuperação motora foi grande e superior à fluoxetina isoladamente. Além disso, encontramos uma assinatura neural diferente do placebo e da EMTr M1 neste ensaio, apoiada por outros ensaios anteriores que conduzimos. Além disso, nosso laboratório esteve envolvido em outros grandes ensaios com efeitos placebo significativos na aprendizagem motora. Esta proposta conduzirá um ensaio experimental no qual 56 indivíduos com AVC crônico serão randomizados em quatro grupos (2:2:2:1): apenas placebo aberto (16 indivíduos), EMTr simulada apenas (placebo oculto) (16 indivíduos), sem tratamento (16 indivíduos) e EMTr M1 isoladamente (8 indivíduos). Os assuntos serão avaliados antes e depois da intervenção usando marcadores neurofisiológicos de conectividade para testar questões mecanicistas específicas com base em nossos dados preliminares. Usaremos análise quantitativa de eletroencefalografia (EEG) de áreas pré-frontais e sensório-motoras e TMS de pulso único e, secundariamente, pareado para avaliar a excitabilidade corticoespinhal e intracortical. Nossa hipótese é que o placebo (aberto e oculto) terá uma assinatura EEG/neural específica caracterizada por um aprimoramento no EEG de assimetria alfa frontal esquerda (FAA) e um aumento da conectividade EEG pré-motora da banda beta. Parkinson's Disease: Enhancing Physical Therapy With Brain Stimulation for Treating Postural Instability., Descrição: Este ensaio tem como objetivo compreender o mecanismo e testar se a estimulação transcraniana por corrente contínua (ETCC) combinada com ultrassom transcraniano (TUS) (tDCS + UST) combinada com fisioterapia (PT) irá induzir efeitos terapêuticos significativos na instabilidade postural na doença de Parkinson (PT ) pacientes. Optimized tDCS for fibromyalgia: targeting the endogenous pain control system, Descrição: A dor da fibromialgia (FM) afeta mais milhares de pessoas pode ter um impacto considerável na qualidade de vida dos pacientes e nos custos para a sociedade (por exemplo, tratamentos ou custos de desemprego). Dados os efeitos limitados e variáveis #8203;#8203;da maioria das intervenções terapêuticas para a FM, existe uma necessidade clínica não atendida para o desenvolvimento de novas intervenções na FM. Evidências recentes sugerem que a dor causada pela FM pode estar relacionada a déficits no controle regulatório endógeno da dor. Neste contexto, a nossa hipótese central é que, para melhorar a sua eficácia, novos tratamentos da FM precisam ter como alvo redes neurais específicas associadas a este sistema endógeno de regulação da dor. Nossa justificativa é baseada em estudos que mostram que a estimulação transcraniana por corrente contínua (ETCC) do córtex motor primário é uma poderosa técnica de neuromodulação não invasiva conhecida por modular significativamente a plasticidade neural e aliviar a dor crônica.Nossos objetivos específicos são: (i) avaliar os efeitos da ETCC e do exercício aeróbico no controle da dor endógena, avaliado pela soma lenta temporal da dor (TSPS) e pela modulação condicionada da dor (CPM); (ii) determinar o efeito dessas intervenções nos marcadores corticais de controle inibitório que também são afetados na FM, como a inibição intracortical avaliada por estimulação magnética transcraniana (TMS) e alterações na disritmia tálamo-cortical avaliada por EEG; (iii) avaliar se o envolvimento dos dois principais alvos testados neste estudo STS e CPM está associado a mudanças nos resultados da dor (por exemplo, Brief Pain Inventory e outras escalas clínicas). Nossa abordagem principal será realizada por meio de um ensaio clínico randomizado 2x2 mecanístico fatorial controlado em 94 pacientes com diagnóstico de FM. Os assuntos serão randomizados para um dos quatro grupos: tDCS e AE; ETCC simulada e EA; ETCC ativa e NAE (não-EA= intervenção de controle para EA); sham tDCS e NAE. As descobertas deste estudo serão significativas, pois existe uma necessidade clínica não atendida para o desenvolvimento de novas terapias na FM, particularmente as não farmacológicas, baseadas em abordagens mecanicistas e no envolvimento do alvo.. , Situação: Em andamento; Natureza: Pesquisa. , Integrantes: Felipe Fregni - Coordenador.

O déficit da inibição como marcador de neuroplasticidade na reabilitação, Descrição: De acordo com a OMS, mais de um bilhão de pessoas no mundo possuem alguma deficiência, o que não é apenas uma questão de saúde pública, mas também de direitos humanos. Em 2015, o Global Burden of Diseases apresentou evidências epidemiológicas mundiais de que 74 do total de anos vividos com incapacidade estão relacionados a condições de saúde que podem ser beneficiadas pela reabilitação. Por essa razão, o desenvolvimento de novas terapias e pesquisas em reabilitação são um dos principais objetivos do Plano de Ação Global da OMS para 2014-2021. No entanto, o pouco conhecimento sobre os mecanismos biológicos envolvidos no processo de reabilitação é uma das principais limitações enfrentadas para a construção de um conhecimento sólido. Há evidencias de que a plasticidade cerebral é o mecanismo central envolvido no processo de recuperação funcional de pacientes com déficit de diferentes etiologias. Embora muito já se tenha avançado no entendimento da plasticidade cerebral, pouco foi incorporado na prática da reabilitação, principalmente pela escassez de desenhos de estudo que privilegiem a translação do conhecimento. Dessa forma, propõe-se um estudo em quatro grupos de sujeitos (com AVC, lesão medular, amputações e osteoartrose) visando a entender os mecanismos de neuroplasticidade envolvidos no processo de reabilitação motora, utilizando para isso as técnicas de EMT, fNIRS e EEG de alta densidade, antes e depois do período de reabilitação. Por meio deste projeto, espera-se obter um melhor entendimento dos mecanismos de neuroplasticidades envolvidos na reabilitação, e o desenvolvimento de biomarcadores neurofisiológicos "transdiagnósticos", os quais terão grande relevância para o aperfeiçoamento científico e terapêutico da reabilitação. NEUROMODULAÇÃO TRANSCRANIANA COMO INSTRUMENTO DIAGNÓSTICO E TERAPÊUTICO, Descrição: O uso da estimulação transcraniana no tratamento da dor e transtornos neuropsiquiátricos tem como suporte teórico evidências recentes, que sugerem reduzir a dor e modificar correlatos neurofisiológicos da experiência dolorosa. Em estudos com ratos conduzidos pelo nosso grupo a ETCC reverteu à dor induzida em modelo de artrite e a hiperalgesia induzida pelo estresse crônico. A partir destas evidências adicionadas à eficácia da ETCC na área do córtex motor primário para tratar síndromes dolorosas, o objetivo principal deste projeto temático será investigar o uso das técnicas de neurmodulação transcraniana como instrumento diagnóstico e terapêutico num conjunto de subprojetos. Trata-se de um projeto temático que envolve seis subprojetos relacionados a técnicas neuromodulatórias cujas metas específicas são: i) Avaliar os processos de neuroplasticidade adaptativa cerebral em pacientes com fibromialgia submetidos à aplicação de ETCC, à administração de pregabalina e à combinação de ambas. O impacto do tratamento será avaliado por meio de desfechos relacionados ao limiar de dor, intensidade da dor, catastrofismo relacionado à dor, qualidade de vida, parâmetros de excitabilidade cortical, acoplamento metabólico vascular (relação oxi-deoxihemoglobina no território cortical correspondente ao território de estímulo periférico usando espectroscopia óptica de neuroimgaem funcional) e marcadores séricos ao efeito do tratamento: agudo, intensivo, na manutenção e pós-tratamento. ii) Avaliar a eficácia da ETCC e da IMS comparada à do tratamento simulado (sham), na redução da dor, na capacidade funcional e no sistema neuroimunoendócrino de pacientes com osteoartrite. iii) Investigar o efeito da relação entre o nível de deslocamento da atenção, parâmetros de excitabilidade cortical, limiar de dor, potência dos sistemas moduladores descendentes e marcadores de atividade neurotrófica em pacientes fibromiálgicas, comparando-as a controles saudáveis, bem como o efeito da ETCC nestes desfechos. iv) Investigar a correlação entre o nível de resiliência, catastrofismo e deslocamento da atenção em pacientes com dor crônica por fibromialgia. Assim como investigar a correlação entre resiliência, dor e sistemas moduladores, sobretudo a relação entre resiliência e marcadores de neuroplasticidade. Adicionalmente, comparar o efeito da ETCC nestes desfechos entre pacientes resilientes e não resilientes e destes com sujeitos saudáveis. v) Comparar a eficácia da TMS na redução da dor, regulação do eixo imune-pineal, função autonômica e pensamento catastrófico. vi) Desenvolver um aparelho de ETTC de baixo custo para uso domiciliar, validá-lo em sujeitos saudáveis e num segundo momento utilizar o aparelho no tratamento de patologias especificas a citar: fibromialgia, transtorno de déficit de atenção e depressão maior.
